I had a similar situation after getting window 7 Home updates installed some time ago but after a cold boot and some refresh's it came back. don't know if it has something to do with Microsoft C++ updates.

Is it only happening when you are using the light grey UI and setting up your own toolbars?
Can you reproduce it with the default dark grey UI?
