I think that the 3D extrude in Xara Xtreme 4 may be even faster than that now .....

I prefer to use Xtreme for this.
Of course depending on the graphics used for the face and spine and how much fiddling you need to do (and how quick you are) will determine your POV.
However as an example, this one took me less than a 3 minutes to finish. (...the mini tutorial much longer but..... )

Nah! I couldn't finish it in 30 seconds. Hell, I can't finish anything in 30 seconds. My purpose was just to toss another program into the discussion. My first attack on such a problem would be to use Xara as well. I do know that some nice box work has been produced by Inkscape and I think it is always worthwhile seeing what other people can do with other programs.

Agreed - though I'm all for keeping it simple.
Inkscape isn't so nice to work with imho and well - I own Xtreme and use it daily.
One amendment to my above process, of course you don't have to clone and slice out the face graphic if you imply create the initial 3D extrude from the face graphic itself and adjust the bevel & lights. The entire face design can be created first then extruded into the box shape.
Shaves some time off the entire process
