Create a fill-in form

I need to have a form where a visitor can leave their name and other info. Can't seem to find the answer in the FAQ search or anywhere else. Any help is greatly appreciated.

In the Designs Gallery > Widgets folder there are some Form Widgets you can use.

Hi Kitty. You might also like to try CoffeeCup Form Builder http://www.coffeecup.com/form-builder/ . It is really easy to use and you can make the forms as comprehensive as you want them to be. And its cheap.
