Verify that the shortcut icon is the correct size and format. For more information, see Creating Icons in the Correct Size and Format. Note that shortcut icons in image formats other than .ico—for instance, .bmp, .gif, or .png—cannot be displayed in Internet Explorer.
I've never used anything else than .ico out of habit.
In Xtreme, I create the 16x16 image, copy it to the clipboard, paste into Irfanview and save as .ico (irfanview allows you to select which colour should be transparent if you need this)
It's quick and simple.
Just used Steve's <link rel="SHORTCUT ICON" href="favicon.ico"> and it works straight away in FF3 & Chrome. IE told me one more thing was loading but when I clicked on an internal link the favicon appeared - on reloading IE the favicon appeared straight away.

Sorry to reopen this year-old thread, but I still have troubles to get a favicon to show up in IE8. I read this whole thread thoroughly, but didn't found a solution (at least, not for me). I'm creating the website in WD6.
This is what I did so far:
- created a favicon in Xara, created a true .ico file with IrfanView
- saved it in the same directory as the index.htm
- created a placeholder, placed it on the pasteboard, named it <head>,
- code for placeholder: <link rel="shortcut icon" href="http://www.camping-leboucher.com/favicon.ico" />
- uploaded the website to the web.
- in IE : emptied the cache, placed the URL in favorites, waited two days, renewed a hundred times.
It shows in all browsers (FF 3.0.4, Chrome, Opera, Avant, Safari) except IE8.
This is the URL www.camping-leboucher.com (not finished)
I hope someone has found the solution in a year's time...
