Creating a Fillable Form

Hi I am trying to find out if it is possible to make a fillable form in Xara web. Basically I want a "Contact Us" for for my clients to place their name, address, comments, etc. I want to build this on my site and have a submit button that will then send me their information, how or can I accomplish this. I have done this before in other programs but have not found any information in this one about it.

Do search on forums form Forms and there is many suggestions. It can be done with the use of placeholders and PHP scripts.
