Hi, Do I need two forms, one for mobile and one for desktop? I have tried one form before and ran into problems with sizing.
My latest website I did one for the width of the mobile device and another for the desktop, should I keep this up?

Yes, one for desktop and one for mobile.
I have found that the loss of user-entered information when you switch to a different variant is not a good experience.
If the form is small enough to be completed on a phone, don't change it for a main site.
I would build a separate form page and use IFRAMEs to access the same form for any other page.

I have two, but that is to see wich one is used the most, the site or the mobile site.
Nice idea. Also you can use a free 3 month website traffic analyzer called Statcounter.
