Printing/exporting a 2 page spread

I know that it can be done - at least in an earlier version! - but how do I get to export to PDF, a 2 page spread. Every time I try the PDF is just a list of single pages.

I think this has to be done in Acrobat.

If you want to go to the trouble, you can create a page that is the full two page size and add your content. Using the Double Page setting does not export as a spread.
