Problem with Form Logix Widget Insert

I'm using Web Designer 10 with my Windows 7 laptop. I have tried to insert the FormLogix e-mail widget (Contact V1) from the Online Content Catalogue. When I complete the required info in the pop up window and then press insert, then the e-mail widget does not appear on my web page. Instead only a white rectangle appears on my web page (appears to be a field where the widget insert is meant to be).

Does anyone know what I need to do to solve this issue?

Generally with widgets and forms, etc., after you have make your additions you need to Copy the content to the clipboard, then press Insert.

Check the Widget code is properly wrapped in <SCRIPT></SCRIPT> tags.
Provide a link or upload the Widget code (making sure there is no account detail included).
