Does the Fast Publish option to only work in the one session...?

I have a fairly large site - 92 pages plus mobile variant.

When I make a change after not having made a change in a while - even a small one - and publish, it still takes a bloody long time to upload 15-20 minutes, and I have cable internet.

I have Fast Publish checked in my FTP settings, but I'm not seeing much evidence that Xara is actually uploading only the changed content.

After the first upload, subsequent uploads go faster, but it seems to do a full upload the first time.

There is a file that is created when you publish that Xara checks when you make changes to determine what needs to be changed. That said, if you add a new page or a lot of new photos and content, this can sometimes effect other pages as well because of the image number naming etc.
