My votes would go for :

    Custom Path Profiles
    Fillet
Inside, center or outside line position

Also a Path based deform too, shapes and/or images that bends along a stroke. (in conjunction with custom path profiles.)

I guess I've said it so many times I feel like a parrot.

    I agree with Osa

Ability to chose between inside, center or outside line position of an Object! (as in AI)

    +1

And how about one more feature advantage offset and inset stroke, you would be able to hold down a shortcut key and drag on the stroke to move the stroke into or outside of the fill.

Fillet is pretty handy tool for creating many kinds of things but for fillet to be really useful and have more flexibility it should be thought of just as an arc with specific radius fitted into something. Below is an example made with CAD software I use to do most of the actual drawing before importing into vector programs.

And while talking about fillet, better not forget chamfer tool also as then would be possible to quickly round some corners and cut others.

I have not used it enough but DrawPlus6 has all the path features that you requested with better performance and interface than Illustrator.
