All forms are from jotform.com they are free (for 100 submissions per month) and the paid price plans are very good as well.

Thanks, skech. We're using Jotforms, too, for our in-house site development and our "first" client (in a few years, anyway). I only asked because your form was VERY clean and I like the way you laid it out.

Another brilliant site from Skech. Even though I am a woman and pink is associated as being female..I hate it! BUT, I think it works very well on this site and my usual repugnancy to the colour pink did not reveal itself at all whilst I was browsing the site.

Thank you abi, I'm not a fan of pink either, if a client leaves the colour choice to us and does not have a corporate colour we will do the site design in various colours but I am shocked by how many people choose the pink version, and not just women; as per the stereotype, but many men as well, they usually like how it makes the site 'pop'.
