Contact Us Form recommendation

Hi

I'd really welcome any recommended widgets for a very simple, but clean & smart looking 'Contact Us' widget form to add to my contacts page.
I'm only after a white background form to capture the following:

Full Name
Company Name
Email Address
Message

All the sites I've looked at so far aren't as easy I'd hoped.

Otherwise I'm v happy with my Xara site so far!

I've had very good luck with JotForm. It is not the easiest to understand but the features are good and the price is right.

There is a JotForm widget in the Designs Gallery > Page Elements > Form Widgets section.

One important step that is not mentioned. Create the form as you want it and then generate the code.

Copy the code to the clipboard (Edit > Copy) then press Insert.

If you don't copy the code first to the clipboard, you will get an error message when you press Insert.

Yep, I am a newbie and coincidentally have just utilised Jotform. Was very straightforward and seems to do exactly what is required!

I made one using the Form2go widget.
Very simple, and works great.
