CoffeeCup Web Form Builder (http://www.coffeecup.com/form-builder/) allows you to set a field as an upload and is easy to use but the issue may well be with your server configuration anyway... see: http://www.coffeecup.com/forums/form...hen-uploading/
Filezilla (http://filezilla-project.org/) is free FTP program that is about as good as it gets. Use that to create a folder and upload your files to it. CoffeeCup Web Access Manager (http://www.coffeecup.com/website-access-manager/) is a snip at $34 and with it you can password protect folders and files as well as set time limits for access.
