How do you get a logo in the address bar?

I've noticed that a lot more websites have a small logo in the browsers address bar, been wondering how this is done.

Any ideas?

I've noticed the favicons sometimes disappear.
Is their anyway to prevent this?

favicon in IE will only appear if the site is a favorite

in other browsers they even appear if they're not a favorite

Yes, I've noticed that too. When they've disapeared and I close IE and open it again they are suddenly back ! How come?

I'm sorry for the flaw in my statement
I should have said that in IE it doesn't allways show unless it's a fav
(so sometimes it doesn't work)
