I think most of them are capable of that.
Photoshop is for sure. A lot depends on the photo quality and the skill level of the artist.
The method would be a cut, paste and layering technique, then some color adjustments, possibly some brush work.

Thats easy, just use the polygonal lasso tool and draw a rough outline around the face but keep inside the edges (just the skin) and then feather your selection about 5 pixels (if at 72dpi) then copy your selection, then paste onto your new pic. Ajust your colors and tone, and your done.
