Welcome to the forum. You have great technique. The old gentleman has great expression.
If I may attempt to show an idea of how to make the arm and hand look aged. Please excuse the poor attempt at the hand [img]/infopop/emoticons/icon_rolleyes.gif[/img]
A few elongate ellipses feathered and mix transparency. Overlay with a clone of the original arm shape and use fractle clouds fill and stainedglass transparency.

Congratulations! Beautiful image!
I understood you wanted to make the book in focus. I would suggest to make a pen and a hand more sharp and to add some light to the neck and the body for to make some details more light then background. Sorry for poor English.
