Sorry to be a pedant but is the Xara Web Properties > Page: Page filename truly Index (as that ought not to be possible)?
It should be index.
Your public folder therefore should have index.htm and index.xhl.
If things are publishing correctly, you should be able to download index.xhl through its URL: ..<websiteUrl>/index.xhl and open it in Notepad or other text editor.
This lists all the published files so you ought to see in index.htm was published.
What does the site do when you open in in a browser?
Are you sure you are not typing in <websiteUrl>/Index.htm or index.html or Index.html in error?

Sorry, it is actually index - all lower case. it's creating index.xhl but i cannot find index.htm or index.html
By default, Xara names the home page index with no extension but publishes as index.htm.
What happens if you edit the name in the Web Properties ? Page section to index.htm
And are you seeing this problem locally on your computer or when you publish to your web host?
And if to your web host are your publishing to the sub-folder (if required) using something like public_html
Finally. Try this. Create a New folder on your desktop and File > Export Website to this new folder. Now do you see the appropriate files?
I cannot find index.htm on the published web site.
Thanks for your help. The file was uploading to an old folder. I've moved it now and all is good.
