Images / layers not printing in CS

Hi, I've been using Photohop for years, but can't figure this one out:

I have a file which has several layers in it, but only the one layer will print. All the layers are visible and editable to work on and the document looks fine in print preview.

I've flattened layers, merged visible, saved as a new document, selected all and printed selection and select all > copy merged and pasted into a new document, but still only this same single layer will print.

Other documents from Photoshop print fine, so it's not a problem with Photoshop as a whole or the printer and it doesn't matter which layer is selected when the document is sent to print.

Any suggestions welcome. Thanks.
