I extensively use the extrude tool in these similar "unusual" ways, though since its the only use of the extrude tool for my work - I don't find it unusual at all. I use extrude for simple map objects for use in some of my map designs. Your book is a good example, and I've done that exact thing for a book in one of my maps. I've never introduced this concept in a thread of my own, as I assumed everyone else was using 3D extrude for the same purposes - as in how you're presenting it here.

Here is an example of some different things that you can do with extrusions. By taking a basic shape and chopping holes in it or cutting it into pieces and grouping the pieces you can then extrude it and by "supersizing" the bevel you can create extrusions that appear somewhat sculpted. In the example below everything started out as ellipses (except the handle of the cup, that started as a curved line)
The coffee beans I adjusted the colours of the lights to achieve the look. The cookies on the plate are an example of what the ability to map or displace a texture on all surfaces of an extrusion could achieve. I used a plug in for this .
Oh and the liquid in the cup is another ellipse and the steam is one of Gary's wisps

Here's a 2.5D scene. The palm trees are just extrudes but in a loose artistic way they work, I think. Plus, when you extrude something, by rotating a copy of it, you can convincingly suggest more than one unique object in the scene.
