Polling Widget

Cannot see (doesn't mean its not there) a widget for Polling. Is there one?

If not can anyone recommend a polling facility please?
Anybody know/recommend LimeSurvey (it comes with cPanel on out web hosting)?

Correction: Jotform widget does include a survey. But, I would welcome any suggestions for alternatives.

LimeSurvey: Took the plunge & installed it on the web hosting service. It's very comprehensive but clunky & not exactly a slick user interface! OK as a last desperate resort.
