Jotform Help

I am having an issue with my form from Jotform publishing. It shows up in WD6 but when I publish my website it simply is not there. This is only happening on one of my forms (I have several on my site). All others work just fine. Please help!

Lucky, shows up ok when I checked it out using google chrome
