I know this thread is older, but if someone stumbles upon it such as I did looking for help, then I wanted to add my two cents worth to assist others.
I had the same problem and could not for the life of me get the "repel text under" to work even though I had successfully done it a million times (slight exaggeration, but a lot)...My issue which I discovered was some how the text box itself had the "repel text under" box checked, which caused the issue. ONLY the objects/images on top of the text should have the repel text box checked. If you are having the same problem and doing every thing to repel text to no avail, definitely try this remedy. It worked like a charm as soon as I unchecked the box!

Yes. Your car probably clipped with a vector mask? And it is not PDF/X?
This problem also exists in the InDesign too. But in InDesign the text can flow around graphics, which is located on the lower layers.
By the way. If the transparent bitmap is placed above CMYK background, so by PDF/X exporting whole bitmap area converted to single bitmap.
