Disabled shift key hand

I can't remember the exact problems I have had with one hand but here is an example:
I need to make a rectangular hole in the orange ellipse as shown on the right.
Egg gave 2 methods
1 - Marquee select.
2 - baby heel on shift key.

Shaman and Gary gave the same method number 1.

If you can afford to loose the context sensitive popup menu on right clicking, how about changing the action of a right click to shift-click?
