The two page spread option is two pages, so for example, if you have a printer that can print on both sides of the page, you can print both pages.
To make a tri-fold brochure, assuming you want to use a single sheet of 8.5 x 11 paper, set the page to landscape, and divide the page into thirds. If you are printing to a deskjet printer, make sure you have the Window > Show Print Borders enabled so you can see what part of the page is not printable.
You might need some trial and error to get the page to back up correctly.

Depending on your design, the paper and how the brochure will be folded, you may need to adjust the widths of the "thirds" so they're not exactly even. More than likely, the panels on the lefthand third of the sheet will need to be slighly wider, and those on the righthand third slightly more narrow to accomodate the fold.
