3D shutter card/Tri-Fold Card

I would like to make a digital version of the card attached to this post.(I hope I did it right) I have created most of it using blocks in individual layers and altering the perspective. My problem is that one end of the card looks like it is falling forward and is not the same width as the other end.

Once I have created the basic outline for the card I would like to use it in a digital scrapbook layout.
